[Pharmacological Actions]
Amiloride is a potassium-sparing diuretic that acts on the distal renal tubule, blocking the sodium-potassium exchange mechanism, promoting sodium and chloride excretion while reducing potassium and hydrogen ion secretion. Its effects are independent of aldosterone, and its own natriuretic and antihypertensive activity is weak. However, it exhibits synergistic effects when used in combination with thiazides (hydrochlorothiazide) or loop diuretics. This combination preparation exhibits synergistic potassium-sparing diuretic and antihypertensive effects. Combining the effects of amiloride and hydrochlorothiazide, it exerts natriuretic and diuretic effects on both the distal and proximal renal tubules. Therefore, this product achieves both potassium sparing and diuretic effects, avoiding the hypokalemic effects of hydrochlorothiazide alone and the weak diuretic potency of amiloride.
