[Properties]

[Indications]
For the treatment of hypertension.
[Dosage and Administration]
Usual adult dose: Oral, 2.5 mg (1 tablet) once daily.
[Adverse Reactions]
Mild, transient, and dose-related. 1. Less common reactions include: diarrhea, headache, decreased appetite, insomnia, nausea, and orthostatic hypotension. 2. Rare reactions include: allergic reactions such as rash and itching; hyponatremia, hypokalemia, and hypochloremic alkalosis.
[Contraindications]
Sulfonamide allergy, severe renal insufficiency, hepatic encephalopathy or severe hepatic insufficiency, and hypokalemia.
[Precautions]
1. To reduce the possibility of electrolyte imbalance, a smaller effective dose should be used. Serum potassium, sodium, and uric acid levels should be monitored regularly to maintain fluid and electrolyte balance and ensure timely potassium replenishment. 2. When used as a diuretic, it is best to administer the drug once a day in the morning to avoid having to urinate at night. 3. Anuria or severe renal insufficiency may induce azotemia. 4. Glucose tolerance may be impaired in diabetes. 5. Gout or hyperuricemia may further increase serum uric acid. 6. Hepatic insufficiency: diuresis may precipitate hepatic coma. 7. After sympathectomy, the antihypertensive effect may be enhanced. 8. If surgery is required while using this drug, it is not necessary to discontinue use, but the anesthesiologist must be informed. 9. Use with caution in athletes.
[Use in Special Populations]
Precautions for children: Not yet established.
Precautions for pregnancy and lactation: Human studies are lacking, and animal studies have not revealed any issues. It is unknown whether this drug is excreted into breast milk, but no issues have occurred in humans.
Precautions for the elderly: Elderly individuals are more sensitive to antihypertensive effects and electrolyte changes, and often experience changes in renal function. Therefore, caution is advised when using this drug.
[Drug Interactions]
1. This product has a reduced diuretic and natriuretic effect when used concomitantly with corticosteroids.
2. This product may cause arrhythmias due to hypokalemia when used concomitantly with amiodarone.
3. This product has a reduced anticoagulant effect when used concomitantly with oral anticoagulants.
4. This product has a reduced natriuretic effect when used concomitantly with nonsteroidal anti-inflammatory analgesics.
5. This product has a greater diuretic effect when used concomitantly with dopamine.
6. This product has a greater antihypertensive effect when used concomitantly with other antihypertensive drugs.
7. This product has a reduced antihypertensive effect when used concomitantly with sympathomimetics.
8. This product may increase blood lithium concentrations and cause signs of overdose when used concomitantly with lithium.
9. When used concomitantly with high-dose salicylates, dehydrated patients may develop acute renal failure.
10. This product may cause lactic acidosis when used concomitantly with metformin.
[Pharmacological Actions]
This is a sulfonamide diuretic that acts by inhibiting water and electrolyte reabsorption in the dilute segment of the distal renal tubular cortex. The mechanism of its antihypertensive effect is unclear, and its diuretic effect cannot explain its antihypertensive effect, as the dose at which it occurs is much lower than the dose at which it produces a diuretic effect. Possible mechanisms include: regulating calcium influx into vascular smooth muscle cells; stimulating the synthesis of prostaglandins PGE2 and PGI2; and reducing vascular hypersensitivity to vasopressors, thereby inhibiting vasoconstriction. This drug has little or no effect on cardiac output, heart rate, and rhythm when used to lower blood pressure. Long-term use rarely affects glomerular filtration rate or renal blood flow.
