[Pharmacological Actions]
Hydrochlorothiazide and triamterene are diuretics that reduce water and sodium retention, decrease blood volume, and reduce circulating blood volume, thereby lowering blood pressure. At the same time, because sodium excretion reduces sodium ion concentration in the blood vessel wall, it weakens vascular responsiveness to catecholamines and angiotensin, thereby enhancing the antihypertensive effect of baseline antihypertensive medications and creating a synergistic effect. The combined use of hydrochlorothiazide and triamterene can enhance diuresis, reduce the dosage of each medication, and antagonize each other's side effects. Hydrochlorothiazide acts on the distal convoluted tubule and the cortical portion of the ascending limb of the loop of Henle, inhibiting sodium ion reabsorption, allowing a larger amount of sodium ions to reach the distal convoluted tubule and collecting duct, resulting in a diuretic effect. Triamterene is a potassium-sparing diuretic with a weaker diuretic effect and can alleviate hydrochlorothiazide-induced hypokalemia. Hydralazine sulfate and reserpine are antihypertensive drugs that dilate arterioles and lower blood pressure. Reserpine can reduce or even deplete the amount of the conduction mediator norepinephrine stored in sympathetic postganglionic fiber terminals, resulting in an inhibitory effect on noradrenergic nerves and a lowering of blood pressure. The combined use of these two drugs has a synergistic antihypertensive effect.
