[Dosage and Administration]
To minimize dose-independent side effects, combination therapy is generally only used when monotherapy fails to achieve satisfactory results. If blood pressure is not effectively controlled with telmisartan 80mg alone, or if blood pressure is uncontrolled with hydrochlorothiazide 25mg once daily, or if blood pressure is controlled but hypokalemia develops, consider switching to telmisartan hydrochlorothiazide capsules (i.e., 40mg telmisartan/12.5mg hydrochlorothiazide), one tablet once daily. The dose can be increased to a maximum of 160mg/25mg if necessary. Telmisartan should be used in patients with hypovolemia after blood volume correction or under close medical supervision. Patients with biliary obstruction or impaired liver function must be initially treated under close medical supervision. Telmisartan Hydrochlorothiazide Capsules can be taken before or after meals, alone or in combination with other antihypertensive medications. Patients with Impaired Renal Function: Conventional treatment with this drug requires a creatinine clearance of 30 ml/min or higher. This drug is not recommended for patients with severe renal impairment, as loop diuretics are often used. Patients with Impaired Liver Function: This drug is not recommended for patients with severe liver impairment. Patients with biliary obstruction or impaired liver function should initiate this drug at a dose of 40 mg/12.5 mg under close medical supervision.
