[Ingredients]
The main ingredient of this product is felodipine, whose chemical name is 4-(2,3-dichlorophenyl)-1,4-dihydro-2,6-dimethyl-3-pyridinedicarboxylic acid ethyl ester methyl ester.
[Appearance]
White or off-white tablets.
[Indications]
For the treatment of mild to moderate essential hypertension.
[Dosage and Administration]
Oral: Starting dose: 2.5 mg twice daily, or as directed by a physician. The usual maintenance dose is 5 mg or 10 mg daily. The dose can be increased if necessary, or other antihypertensive medications can be added.
Adverse Reactions:
1. Like other calcium channel blockers, this drug may cause flushing, headache, dizziness, palpitations, and fatigue in some patients. Most of these reactions are dose-dependent, begin within a short period of time after the dose is increased, are transient, and disappear with prolonged use.
2. Like other dihydropyridines, this drug may cause dose-related ankle swelling and mild gingival swelling in patients with gingivitis or periodontitis.
3. Rash and itching may also occur.
4. In rare cases, significant hypotension with tachycardia may occur, which may cause myocardial hypoxia in susceptible individuals.
Contraindications:
This drug should not be used in patients with allergies. It should not be used during pregnancy.
Precautions:
Overdose may cause excessive peripheral vasodilation, accompanied by significant hypotension and, in some cases, bradycardia. Symptomatic treatment should be given if severe hypotension occurs, such as placing the patient supine with the legs elevated. If bradycardia is present, intravenous atropine 0.5-1.0 mg should be administered. If this is ineffective, glucose, saline, or dextran should be infused to expand blood volume. If the above measures are ineffective, sympathomimetic amines that primarily act at α1-adrenergic receptors may be administered.
[Special Population Use]
Precautions for Children: No experience exists for use in children.
Precautions for Pregnancy and Lactation: Contraindicated in pregnant women.
Precautions for Elderly: No experience exists for use in the elderly. Please refer to the adult dosage or follow your doctor's advice.
[Drug Interactions]
1. Concomitant use of cytochrome P450 inhibitors with this drug may affect felodipine blood levels. 2. Enzyme-inducing drugs (such as phenytoin, carbamazepine, and barbiturates) may decrease felodipine blood levels. 3. Enzyme inhibitors (such as cimetidine) may increase felodipine blood levels. Although felodipine has a high degree of plasma protein binding, it does not affect the binding of other plasma protein-bound drugs (such as warfarin).
[Pharmacological Action]
This drug is a selective calcium antagonist that primarily inhibits extracellular calcium influx into arteriolar smooth muscle cells, selectively dilating arterioles without affecting veins. It does not cause postural hypotension and has no significant inhibitory effect on the myocardium. While reducing renal vascular resistance, it does not affect glomerular filtration rate and creatinine clearance, and renal blood flow remains unchanged or may even increase slightly. It has natriuretic and diuretic effects. It increases cardiac output and cardiac index, significantly reduces afterload, and has no significant effect on cardiac systolic function, preload, or heart rate.
