[Pharmacological Actions]
Dipyridamole has antithrombotic effects. It inhibits platelet aggregation and, at high concentrations (50 μg/ml), can inhibit platelet release. Possible mechanisms of action include: 1. Inhibition of adenosine uptake by erythrocytes, which is a platelet inhibitor; 2. Inhibition of phosphodiesterase, which increases cyclic adenosine monophosphate (cAMP) in platelets; 3. Inhibition of thromboxane A2 (TXA2) formation, which is a potent agonist of platelet activity; and 4. Enhancement of endogenous progesterone phosphatidylcholine (PGI2). Dipyridamole has a vasodilatory effect in dogs. Intraduodenal administration of dipyridamole at 0.5-4.0 mg/kg produces a dose-related decrease in systemic and coronary vascular resistance, lowering systemic blood pressure and increasing coronary blood flow. The onset of action is 24 minutes after administration, and the effects persist for approximately 3 hours. Similar effects are observed after intravenous administration of 0.025-2.0 mg/kg.
